We've been busy making Rural Health Connect even better, and we want to share what's new with the communities we serve across all 15 Area Development Districts in Kentucky.

What Is Rural Health Connect?


Rural Health Connect is a free tool designed specifically for rural Kentucky residents — helping you track your health, connect with local care, and make sure nothing falls through the cracks. Whether you're managing diabetes, high blood pressure, or just trying to stay on top of your checkups, this app was built with your daily life in mind.


What's New


Your Health Trends, Visualized

We've added simple charts that show how your key health numbers — like blood sugar, blood pressure, and cholesterol — have changed over time. Seeing the trend matters just as much as a single reading, and your care team can now see it alongside you.



Help Getting Your Medications Approved

Getting a new medication approved by insurance can feel like a maze. We've added a Prior Authorization tracker so your care coordinator can follow your case step by step — and you can see exactly where things stand, from submission to approval.


Now Available in Spanish

Rural Health Connect now works in both English and Spanish. If Spanish is your preferred language, you can switch at any time, and everything will follow along.


Your District, Your Standards

We know that health needs vary from one part of Kentucky to another. Local health coordinators can now set clinical goals that reflect the specific needs of your community — not just a statewide average.

Rural communities have always shown resilience. Our goal is to make sure the tools available to you are as strong as the people using them. Every update we make is driven by conversations with residents, care workers, and community leaders across Kentucky.
